504/Annual review meetings
Students receiving support through Section 504 of the Rehabilitation Act of 1973 will have an annual review meeting with the campus 504 coordinator. Students that started receiving support through 504 in elementary school or middle school will continue support in high school. Records within the school district will transfer automatically. Students transferring from another district should request a copy of their current plan from the previous district and share with LTHS when they enroll.
Parents often ask to meet at the beginning of the school year to adjust accommodations. In order to make appropriate changes to the 504 plan, the coordinator will request feedback from the student, parents, and current teachers. This process takes some time, but will be as efficient as possible. If you feel that the current accommodations are not appropriate, requests for changes should be made as soon as possible rather than waiting for the next school year.
Request for records
If you are currently a student at LTHS or you are transferring to another school district this year, fill out this form to obtain a copy of your current 504 plan.
If you graduated from LTHS or withdrew from LTHS, contact Nickie Moreno at firstname.lastname@example.org to obtain a copy of your most recent 504 plan. Once you are 18 years old, you must make the request. Records cannot be released to your parents without your consent.
College Board/ACT Accommodations
Students receiving 504 support have the opportunity to request accommodations for the PSAT, SAT, AP, and ACT tests. Accommodations can only be made available to students if requested and approved in advance by College Board and ACT. If you are interested in requesting accommodations for your student, please complete the following Google form AND consent forms and return to the 504 Coordinator, Angela Hrapchak, at email@example.com.
